MEDICATION ADMINISTERED DURING SCHOOL HOURS
No medication (prescription or OTC) will ever be administered at Our Lady of Guadalupe Parish School to a student without prior written permission from the parent & physician. Under California Law, if it is necessary for the school to administer prescription medication to a student, written instructions from the child's physician must be provided each year to the school office. Authorization forms may be picked up at the school office. Medication left over from the 2010-2011 may be picked up and replaced. Medication not picked up by September 30 will be discarded per California Law.
EMERGENCY CARDS
Emergency cards were sent home last week. Please fill it out completely and return to the school immediately, if you have yet to do so.
WALKER AUTHORIZATION FORM
Students walking home must have a walker authorization form on file. No student will be allowed to walk home without a current authorization on file for his or her safety.
HOT LUNCH PROGRAM
Students wishing to purchase hot lunch must buy a lunch card from the school office. The cost of a 10 lunch card is $40.00 dollars or 5 lunches for $20.00 dollars. If you are dropping off a lunch for your child please make sure that it is clearly labeled with their name and grade. Lunch is from 12:00pm to 12:30pm for grades K-4, and 12:30pm-1:00pm for grades 5-8.
